Service Dog Registration for Calvin

I\’m very confused – Calvin had to be put down 6 months ago due to dementia and physical issues.  I was paired with a dog in OK. I don\’t know how to change out the Calvin\’s card for Sissy ?  The stuff I ordered doesn\’t seem to pick up on the new registration.  I don\’t know how to move forward.  If someone could call me ?  I need help.  I\’m still in need of visual assistance but now I have been diagnosis of PV – a blood cancer and osteoarthritis (shoulders are the worst).  On many medications and looking for a vest that can carry pills and a timer if needed too.
