Service Dog Registration for Mr. Myrtle Sir

1. Mr Myrtle Sir is approximately 43 pounds, and approximately 22 inches long. He has a white heartshaped birthmark on his belly, a light patch on his tail, and very distinctive large mogwai ears.

He helps me manage CPTSD and many related things; severe chronic pain issues (due to fibromyalgia and to multiple injuries) including awareness and mobility; and to live with other physical issues

Mr Myrtle also has his own CPTSD – he has been attacked by dogs as well as other animals, and abused by humans… He is dear and brilliant and devoted, he just needs to be understood. He is my service and support dog, I am his service and support human.
